Trumbull County Board of DD, like each County Board in Ohio, is responsible for ensuring the health and safety of those we serve. When there are circumstances that pose a threat to the health, safety or well-being of an individual with a developmental disability; we refer to these incidents as Unusual Incidents (UI) or Major Unusual Incidents (MUI).
In MUI circumstances affecting adults, we have dedicated staff who look into allegations. The Investigative Agents (IA’s) look into the situation to determine what happened (investigation), what caused the event (cause and contributing factors), and work with individuals and their teams to put preventive measures in place to avoid future risk in similar circumstances (prevention plan). In instances of suspected criminal nature, IA’s will refer cases to local law enforcement. In circumstances affecting children, cases are referred to Trumbull County Children Services.
What is an MUI?
An MUI, or Major Unusual Incident, refers to an alleged, suspected, or actual occurrence that raises concerns about the health or welfare of an individual, indicating that they may be at significant risk of harm.
What is a UI?
A UI, or Unusual Incident, is an alleged, suspected, or actual event that deviates from established policies, procedures, or an individual's care plan. However, it does not present the same heightened risk to an individual's health and safety as an MUI. Examples of UIs include falls, dental injuries, and minor medication errors that do not pose a significant health risk. In such cases, your provider or SSA will investigate the contributing factors that led to the incident and work with you to develop a prevention plan.
Who can report an MUI or UI?
Incidents can be reported by family members, friends, providers, or the individuals themselves. When reporting Major Unusual Incidents to the Trumbull County Board of Developmental Disabilities, please reach out to the HELP NETWORK at:

An on-call Investigative Agent will promptly follow up on all reported MUIs.
Upon the identification or notification of a Major Unusual Incident, the Trumbull County Board of Developmental Disabilities will take all reasonable measures to ensure the health and welfare of at-risk individuals.
What happens during an MUI investigation?
Once an MUI is reported, it is assigned to an Investigative Agent (IA), who initiates an administrative investigation. This process includes interviewing those involved to collect facts and statements, as well as reviewing any pertinent documentation.
